10.08 Haemonc [PDF link]: Blood products – Packed red cells (RCC), Platelets, CMV-ve and irradiated blood, fresh frozen plasma (FFP), cryoprecipitate, Prothrombin complex, Cell saver devices. Blood product transfusion complications: acute haemolytic reaction, non-haemolytic febrile reaction, minor allergic reaction, anaphylactic reaction, transfusion-related acute lung injury (TRALI), transfusion-associated circulatory overload (TACO). When to measure INR?
Measure on 3rd/4th day after starting warfarin, then daily or alternate days until INR is within target range (usually between 2-3)
Then twice weekly for next 2 weeks
Then weekly until at least 2 readings within therapeutic range
Then at longer intervals (per local policy / consultant / anticoagulation team)
